Bio-Identical Hormone Replacement Therapy – Common Medical Protocols for Women/Female Patients.

Female patients often ask what some of the more common female patient Hormone Replacement Therapy protocols are. While it’s important to emphasize that every patient can vary, the hormone protocols shown below can provide a broader context for how our HRT Doctors provide treatment.

What Are BioIdentical Hormones?

All of our female protocols use BioIdentical Hormones.
None of our female protocols use Synthetic Hormones.

BioIdentical hormones are identical in molecular structure to the hormones women make in their bodies. They’re not found in this form in nature but are made, or synthesized, from a plant chemical extracted from yams and soy. The most significant consideration is that your body can’t distinguish bioidentical hormones from the ones your ovaries produce.

Unlike bioIdentical hormones, synthetic hormones do not have the exact chemical and molecular structure as the hormones made in your body. As a result, this difference causes the body to react to synthetic hormones.

Female Balance 

This treatment plan is for women at any stage of life that need improved hormonal peaks and peak timing to balance the roles of Estrogen and Progesterone in your body on a monthly basis.  Dosing is biorhythmic, our patient care team will work with you to create a customized calendar so you’ll know exactly how much cream to apply, and which type, each day of the month.

  • Estradiol Cream 10mg/mL
  • Progesterone Cream 200 mg/mL Topi-Click 30mL

A metered dispenser of bio-identical Estradiol cream is used to treat twice daily on the upper torso. Daily dosing is variable and designed to reinforce the appropriate timing of the estrogen peak in a healthy female menstrual cycle. Care should be taken for at least two hours after treatment to avoid contact of the treated areas with men or male children to reduce the risk of progesterone transference.

A metered dispenser of bio-identical Progesterone cream is used to treat twice daily on the thighs or buttocks starting on the 15th day of the menstrual cycle and continuing until your period begins. Daily dosing is variable and designed to reinforce the appropriate timing of the progesterone peak in a healthy female menstrual cycle. Care should be taken for at least two hours after treatment to avoid contact of the treated areas with men or male children to reduce the risk of progesterone transference.

Female Balance Plus                                                                                                                                                                        

This treatment plan is the same as Female Balance, but with the addition of a low dose of Testosterone is added to the Estrogen cream for women that test low for testosterone.  Low testosterone levels in women pose the same health hazards as they do for men.  The testosterone in this plan helps support your bone, tissue, and sexual health.

Female Dominance Plus 

This treatment plan is intended for women that present symptoms and lab results that indicate an imbalance between the Estrogen and Progesterone in their bodies.  This is known as Estrogen Dominance.  For pre-menopausal women this can be particularly problematic, causing sever PMS symptoms, incomplete menstruation, and inconsistent period timing and duration.  In this formula 80% of the Estradiol is replaced with Estriol, a less potent estrogenic hormone that is more easily metabolized by your body, helping to get your Estrogen and Progesterone levels more closely aligned.  Testosterone is also included at low dosage to support bone, tissue, and sexual health.

Bio-Identical Hormone Replacement Therapy For Non-Cyclic, Post-Menopausal Women

Non-cyclic post-menopausal women have much to gain from our HRT program below.  It is not a coincidence that a healthy female cycle is 28 days.  It’s lunar.  So that’s what we do for post-menopausal women, is that we restore the lunar biorhythm.  Day 1 is on the new moon, day 14 the full moon, day 28 is the end of the lunar cycle.  This benefits bone, tissue, brain, and sexual health for our female patients.  The protocol would either be Female Balance or Balance Plus (which adds testosterone) depending on symptoms and labs.  E2 is ramped up and down for the full 28 days, Progesterone is used only during the last 15 days, with both dropping to minimal dosing by day 28.  See below for a typical hormone replacement therapy treatment calendar.  The calendar is typical of what we give to patients and reference clicks of each hormone for ease of use.  Note that one click is .25 mL of cream.
